Puppy Questionnaire

It is my sincerest goal to place all puppies in the best home, suited for their individual needs and the desire of the new owners/family.  These questions are designed to assist for the betterment of all.  All interested must fill out questionnaire.  I respond to all inquiries within a day or two.











  1. Please share if you own or rent your home?  
  2. How long have you lived or will be living at this address?  
  3. How large is your yard/or how many acres?  
  4. Is your yard or property fenced for dogs?  
  5. If so, what type of fencing, how tall?  
  6. If you do not have fencing, how will you handle the puppy's exercise and toilet duties?  
  7. How many adults and children are living or visit your home?  
  8. Ages of children?  
  9. Do you own or plan to have other dogs?  
  10. If yes, describe breed, sex and age:  
  11. If you own other dog(s) are they neutered or spayed?  
  12. If you own a dog(s) and/or cat(s) please give the name, address, and phone number of your veterinarian:  
  13. How many dogs have you owned in the last ten years?
  14. If you have owned dogs in the last ten years that are not presently living with you, what happened to dog(s) and be specific:
  15. Have you owned an Australian Shepherd in the past?
  16. If so, provide, name and contact information of the breeder, rescue or other wise in which you acquired your puppy:
  17. What Dog Clubs are you affiliated with?
  18. Why did you choose this breed?
  19. Is the color of the dog important to you and why?
  20. Do you prefer a Male or Female? Why?

    Would you consider a puppy of another color or sex if your preferences are not available?

  22. Will the puppy reside in the house?
  23. If you work, how will the puppy spend its days?
  24. How will the puppy spend the night? Crated or uncrated, location in your home?
  25. How many hours on average will the dog spend alone per day?
  26. List all plans for this dog: Pet, Herding, Obedience, Conformation, Agility, Breeding, Guard, Other:
  27. Have you put any titles on any of your previous dog(s)?  If yes, list the titles and registered name of the dog(s):

  29. All of our pet/companion puppies are sold on ASCA/AKC Limited Registration and spay/neuter agreements.  Are you comfortable with this? If no, why?
  30. All show/performance prospects are sold on co-ownership.  Are you comfortable with this arrangement? If no, why?
